____ House and household insurance premiums on your personal residence were not deducted.
____ If you used the standard mileage rate to compute your automobile expense deduction, you did not deduct your automobile insurance expense.
____ You did not deduct automobile insurance expense for the amount attributable to personal use.
____ Premiums paid on life insurance policies were not deducted.
____ Deductions and/or payments for credit liability insurance or insurance guaranteeing lines of credit were not deducted.
____ As the direct or indirect beneficiary of the life insurance policy on an officer or employee, you did not deduct the premiums.
____ You did not establish that the amount deducted was insurance expense and was paid.
