Reportable Income - Unearned Income

    You have reported on your tax return:

    ____ Alimony or separate maintenance payments you received.

    ____ Payments made to a third party on your behalf that qualify as alimony or separate maintenance.

    ____ Funds you embezzled.

    ____ Your total gambling winnings.

    ____ Prizes and awards you won including those given for outstanding scientific, educational, artistic, literary or civic achievements

    ____ All interest income unless specifically exempted by law.

    ____ The pension you received.

    ____ The annuity you received.

    ____ The corporate funds you received as dividend income.

    ____ Your dividend income as reflected by the amounts shown on your Form 1099-DIV.

    ____ All capital gain dividends you received.

    ____ The fair market value of property received as a dividend from a corporation.

    ____ The amount of a liquidating distribution you received that is more than your basis in the stock.

    ____ Any profit from the sale of state or municipal securities.

    ____ Any distribution you received out of earnings and profits of the distributing corporation, because it is taxable as a dividend.

    ____ Income from series EE United States Savings Bonds issued after 1989 to finance the higher education of you, your spouse or your dependents that did not meet the filing status or the adjusted gross income limitations or which were not used for higher education of you, your spouse or dependents.
